OPRA Request Form

The Open Public Records Act provides procedures for the public (the "requestor") to review or receive copies of government records. 

To access government records, a requester must determine which records he or she wishes to see or copy.

OPRA applies to requests for records, not requests for isolated facts. 

All requests for records must be as specific as possible.
Requests containing the phrase "Any and All" will be considered too broad and will be denied.

In order to be considered a valid request under OPRA, the request must identify the specific record(s) desired, and a general date in which the record was created. The request must be submitted to the records custodian of the public agency that has the record, and that agency's records request form must be used to ask for the record.

Example of a valid request: "All construction and building permits regarding the instillation and/or demolition of a roof for 123 Main Street, Magnolia, NJ 08049 from 2022 to present"

Example of an invalid request: "Any and all records for the blue house on the corner of Main Street."
